- Appointment required. Merchant contact information will be available for you after purchase in your voucher.

- All patients must bring valid photo identification and Medicare cards, which will be scanned into our system. Patients who fail to provide these documents will not be seen.
- 1 purchase per person only.
- Consultation is 15 minutes only.
- Only eligible for patients with Medicare.
- NO GAP for patients with health insurance.
- Before a regular cleaning can be done, the dentist will check if a deep cleaning is needed. A deep clean is done under anesthetic and treated below the gums. A deep clean is a different procedure and will be quoted accordingly if required. If a deep clean is required, it must occur before the regular clean can occur.
- This voucher is a check-up and clean, and meant for patients who regularly attend for a 6-month check-up and clean. Patients who have not had a clean in over 12 months will most likely be told they need a deep clean before proceeding to a regular general 6-monthly clean.
- A $50 surcharge is required for weekend appointments.
