* Phone Bookings Only, strictly no Health engine / Online -Appointments.
* Limited appointments are available on Saturdays. The latest weekday appointment is at 5:00 pm.
* 48-hour cancellation notice applies else the voucher is forfeited. You need to fill out the registration form, so you will have to come 10 minutes earlier. Running late will also forfeit the voucher.
* Treatment time may vary for each patient. Results may vary for each patient. Any additional treatment including additional scale and cleaning will incur an additional fee.
* Valid for ages 16 and over (unless accompanied by a legal guardian and deemed suitable by the Dentist). The value of the voucher cannot be claimed through your health fund. Any Additional Treatments are paid directly to the merchant.
* Results may vary for each person. Expiry is the suggested date by which your initial consultation should occur, however, your voucher will continue to remain valid and can be used after this date.
