• Full-refund if practitioner deems client unsuitable for procedure or advises against procedure for any reason
• Actual number of units administered is at practitioners discretion
• Not suitable for those who are pregnant, breast feeding, have neuromusular disease, bells palsy or any active skin disease

• You are required to bring your medicare card for the health check prior to cosmetic treatment. If you do not have a medicare card, the health check will be charged at $35

• Please note, Cosmétique will not be handling refunds. Clinical images are to be taken at time of the follow up appointment. Follow up appointment will be between 10-20 days
• Further treatment can be administered at the time of follow up appointment. Offer does not include additional top ups. Any further treatment will need to be paid for
• Clinical images may be taken before and after procedure. These images may be de-identified for marketing purposes pending patient permission
• Majority of units will be used at the first appointment. The remaining can be used at the second appointment post assessment to treatment. Any remaining wrinkle relaxer will be held against your account as a credit and can be used at any time by the patient. The patient cannot be discharged home with wrinkle relaxer
