• Bookings are essential and must be at least 24 hours in advance.
• State Voucher and Security Code at time of booking.
• Present printed or smartphone voucher on arrival.

• 48-hour cancellation policy or voucher is forfeited
• Please arrive 10 minutes early.
• Results may vary for each person.
• Any additional (including subsequent clean) treatment identified as necessary before undergoing treatment will incur an additional cost.
• This service cannot be reimbursed by your Health Fund as the invoice or receipt cannot be provided by the Practice
• Should any X-rays be taken these remain the property of the Practice and if the patient wishes to take a copy, this will be at an additional cost of $50
