• No refund if practitioner deems client unsuitable for procedure or advises against procedure for any reason after the 3D animation stage. Voucher value can be used toward any other orthodontic treatment if dentist advises that a suitable substitute exists
• No refunds given once impressions and records are taken for 3D animation stage • Duration and number of treatments may vary by person
• This service may or may not be covered by your health fund. Refer to your health fund for more information on claiming orthodontic treatment
• Any additional treatment identified as necessary before undergoing treatment will incur an additional cost
• Patients must be free of tooth decay and gum disease prior to the treatment
• Mid-Course correction and refinement, if needed, incurs an additional cost
• Lost or broken aligners will incur a fee
• Upper and lower retainers must be worn after Invisalign® treatment or teeth will relapse. 2 retainers will incur an additional cost of $400
• Patients may be referred to a radiologist for additional X-rays to determine eligibility for procedure; May be covered by Medicare or private health insurance - otherwise surcharge applies
• Expiry is the suggested date by which your initial consultation should occur by, however, your voucher will continue to remain valid and can be used after this date
